To compete in any sport, the athlete and parents/guardians must complete the Two Rivers HS Athletic Paperwork, the WIAA Sports Physical Form, the WIAA Concussion and Cardiac Arrest Form, and read and understand the Athletic Code of Conduct and Athletic Code Meeting.
Parents/guardians must fill out these forms once a year for each student. You may call the Athletics Office (920) 793-2291 if you have any questions.
A sports physical form needs to be completed every two years, and a sports alternate card needs to be filled out every other year.
All freshmen and new students at Two Rivers High School and their parents/guardains must attend the Athletic Code Meeting. Returning athletes and their parents/guardians are not required to attend the meeting, but must read and understand the Athletic Code and complete the form before participating in a sport.
